Core Membership Level
Covenant Core is a voluntary membership offering three Personal Responsibility Amount Yearly (PRAY) tiers and pay-as-you-go pricing with robust features. The Core level provides access to doctor office visits, urgent care, specialty care and hospital services through member-to-member sharing.
Crisis Membership Level
Covenant Crisis is a voluntary membership offering two Personal Responsibility Amount Yearly (PRAY) tiers and pay-as-you-go pricing for catastrophic services. The Crisis level provides access to ambulance, hospital, surgical and maternity services through member-to-member sharing.
Daily Membership Level
The Covenant Daily level provides access to doctor office visits, urgent care, specialty care and services through member-to-member sharing.
Choice Membership Level
The Covenant Choice level provides access to doctor office visits, urgent care, specialty care and services through member-to-member sharing.
